Based on an Occupancy Survey completed on September 25, 2025, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for an existing Nursing health care occupancy.

This Final Occupancy Survey was conducted for alterations to expand the existing LTAC located on the fifth floor of the hospital. The expanded facility will now also have beds located on the sixth floor for a total of 38 beds.

Areas Inspected: Sixth Floor expansion and Penthouse.

Refer to Drawing Number: H-24-1187.

This is a six-story, Type II (222), fire resistive building, with a penthouse and basement, that is fully sprinklered.
