Based on a Relicensure Survey completed on October 23, 2025, it was determined that Emmanuel Center for Nursing and Rehabilitation at Maria Joseph Manor was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy.

This is a one story, Type V (111), protected, wood frame building, with a partial basement, that is fully sprinklered.




 Plan of Correction:


NFPA 101 STANDARD Subdivision of Building Spaces - Smoke Barrie:State only Deficiency.

Smoke barriers shall be constructed to a 1/2-hour fire resistance rating per 8.5. Smoke barriers shall be permitted to terminate at an atrium wall. Smoke dampers are not required in duct penetrations in fully ducted HVAC systems where an approved sprinkler system is installed for smoke compartments adjacent to the smoke barrier.

Observations:

Based on observation and interview, it was determined the facility failed to maintain smoke barrier separation walls in two locations, affecting two of ten smoke compartments.

Findings include:

1. Observation on October 23, 2025, between 11:44 a.m., and 11:46 a.m., revealed the following:

a. 11:44 a.m., the smoke barrier separation wall lacks required half-hour integrity closest to Resident Room 135, due to a sheet metal patch that measures approximately 20 inches by twenty inches.
b. 11:46 a.m., a conduit penetration of the smoke barrier separation wall, located above the smoke barrier doors, closest to Resident Room 135.

Exit interview with the Facilities Manager on October 23, 2025, between 12:30 p.m., and 12:35 p.m., confirmed the smoke barrier separation wall deficiencies.





 Plan of Correction - To be completed: 12/06/2025

1. Deficiencies related to fire rated sealant to penetration areas were corrected by facility maintenance on October 24, 2025.
2. Facility designee to conduct facility wide audit of fire rated sealant to penetration areas to ensure compliance.

Based on observation and interview, it was determined the facility failed to maintain emergency electrical systems in one location, affecting one of one floor.

Findings include:

1. Observation on October 23, 2025, at 12:10 p.m., revealed the remote annunciator was blocked by a cart at the Nurse's Station location.

Exit interview with the Facilities Manager on October 23, 2025, between 12:30 p.m., and 12:35 p.m., confirmed the remote annunciator deficiency.



 Plan of Correction - To be completed: 12/11/2025

1. Deficiency was corrected by facility designee day of inspection; facility assured emergency electrical systems were unobstructed.
2. Facility designee to conduct facility wide audit of access to emergency electrical systems areas to ensure compliance.
