Based on document review and interview, it was determined the facility failed to ensure policies and procedures were in place addressing patient population. Including, but not limited to: persons at-risk, the type of services the facility has the ability to provide in an emergency, and continuity of operations, including delegations of authority and succession plans, affecting the entire facility.

Findings include:

1. Document review on April 9, 2025, at 8:25 a.m., revealed the facility's Emergency Preparedness Plan did not include policies and procedures addressing patient population, including, but not limited to persons at-risk.


Interview with the Facility Administrator and Maintenance Director on April 9, 2025, at 1:00 p.m., confirmed the lack of documentation.

All new personnel must be oriented and assigned specific responsibilities regarding the CORF's emergency plan within 2 weeks of their first workday. (v) If the emergency preparedness policies and procedures are significantly updated, the CORF must conduct training on the updated policies and procedures.

*[For CAHs at §485.625(d):] (1) Training program. The CAH must do all of the following:
(i) Initial training in emergency preparedness policies and procedures, including prompt reporting and extinguishing of fires, protection, and where necessary, evacuation of patients, personnel, and guests, fire prevention, and cooperation with firefighting and disaster authorities, to all new and existing staff, individuals providing services under arrangement, and volunteers, consistent with their expected roles.
(ii) Provide emergency preparedness training at least every 2 years.
(iii) Maintain documentation of the training.

(A) When a community-based exercise is not accessible, conduct an annual individual facility-based functional exercise; or
(B) If the hospice experiences a natural or man-made emergency that requires activation of the emergency plan, the hospice is exempt from engaging in its next required full-scale community based or facility-based functional exercise following the onset of the emergency event.
(ii) Conduct an additional annual exercise that may include, but is not limited to the following:
(A) A second full-scale exercise that is community-based or a facility based functional exercise; or
(B) A mock disaster drill; or
(C) A tabletop exercise or workshop led by a facilitator that includes a group discussion using a narrated, clinically-relevant emergency scenario, and a set of problem statements, directed messages, or prepared questions designed to challenge an emergency plan.

(B) If the [LTC facility] facility experiences an actual natural or man-made emergency that requires activation of the emergency plan, the LTC facility is exempt from engaging its next required a full-scale community-based or individual, facility-based functional exercise following the onset of the emergency event.
(ii) Conduct an additional annual exercise that may include, but is not limited to the following:
(A) A second full-scale exercise that is community-based or an individual, facility based functional exercise; or

Based on a review of the facility's Emergency Preparedness (EP) Plan, it was determined the facility failed to maintain documentation for the two required exercises to test the emergency plan.

Findings include:

1. Interview and documentation review on April 9, 2025 at 9:25 a.m., revealed the facility lacked documentation for the two exercises required annually to test the emergency plan.

Interview with the Facility Administrator and Maintenance Director on April 9, 2025, at 1:00 p.m., confirmed documentation for the two exercises were not available at the time of the survey.

Based on a Medicare/Medicaid Recertification Survey completed on April 9, 2025, it was determined that Carnegie Park Post-Acute, was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a three-story, Type II (111), protected noncombustible building, with a basement, that is fully sprinklered.








 Plan of Correction:


NFPA 101 STANDARD Sprinkler System - Maintenance and Testing:This is a less serious (but not lowest level) deficiency and affects more than a limited number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status. This deficiency was not found to be throughout this facility.
Sprinkler System - Maintenance and Testing
Automatic sprinkler and standpipe systems are inspected, tested, and maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintaining of Water-based Fire Protection Systems. Records of system design, maintenance, inspection and testing are maintained in a secure location and readily available.

Based on observation and interview, it was determined the facility failed to maintain the automatic sprinkler system in two instances, affecting two of eight smoke compartments.

Findings include:

1. Observation on April 9, 2025, revealed the following automatic sprinkler system deficiencies:

a) 10:30 a.m., there were multiple missing ceiling tiles in the basement Mechanical room, which would allow the passage of heat and smoke, and may affect operation of the automatic sprinkler system;
b) 10:40 a.m., there was a ceiling tile that had an excessive gap over 1/8" in the Phone/Data room.

Interview with the Facility Administrator and Maintenance Director on April 9, 2025, at 1:00 p.m., confirmed the automatic sprinkler system deficiencies.







 Plan of Correction - To be completed: 05/17/2025

Maintenance Director repaired both areas of penetration on 4/10/2025. Due to the holes being caused by an outside contractor, the maintenance department will monitor all contracting work and educate them to make sure that all holes are fixed prior to the job be completed.
NFPA 101 STANDARD Electrical Systems - Receptacles:This is a less serious (but not lowest level) deficiency and is isolated to the fewest number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status.
Electrical Systems - Receptacles
Power receptacles have at least one, separate, highly dependable grounding pole capable of maintaining low-contact resistance with its mating plug. In pediatric locations, receptacles in patient rooms, bathrooms, play rooms, and activity rooms, other than nurseries, are listed tamper-resistant or employ a listed cover.
If used in patient care room, ground-fault circuit interrupters (GFCI) are listed.

Based on observation and interview, it was determined the facility failed to maintain electrical receptacles in one instance, for one of over 30 receptacles inspected.

Findings include:

1. Observation on April 9, 2025, at 11:00 a.m., revealed an electrical outlet, located within six feet of a clothes washer in the Occupational Therapy room, was not protected by ground fault circuit interrupter (GFCI).

Interview with the Facility Administrator and Maintenance Director on April 9, 2025, at 1:00 p.m., confirmed the electrical receptacle deficiency listed above.





 Plan of Correction - To be completed: 05/17/2025

the Maintenance Director replaced the plug with a GFI plug as required. The maintenance department will implement a building audit to ensure all appliances are properly placed into the correct plug within the next two weeks.
