Initial comments:

Based on a Follow up Survey completed on February 12, 2025, it was determined that Accela Rehab And Care Center At Somerton, corrected the state deficiencies cited during the survey of December 23, 2024, under the requirements of the 28 Pa. Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations related to the health portion of the survey process.
