Initial comments:

Based on a Revisit Survey and State Monitoring Survey completed on September 6, 2023, it was determined that Guy And Mary Felt Manor, Inc. corrected all the federal deficiencies cited during the survey of July 7, 2023, under the requirements of 42 CFR Part 483, Subpart B Requirements for Long Term Care Facilities; however, the facility was not in compliance with the 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations as they relate to the Health portion of the survey process.




 Plan of Correction:


§ 211.12(f.1)(2) LICENSURE Nursing services. :State only Deficiency.
(2) Effective July 1, 2023, a minimum of 1 nurse aide per 12 residents during the day, 1 nurse aide per 12 residents during the evening, and 1 nurse aide per 20 residents overnight.

Observations:

Based on a review of nursing staffing hours and staff interview, it was determined that the facility failed to ensure a minimum of one nurse aide per 20 residents on three of six overnight shifts reviewed.

Findings include:

Review of nursing staff care hours provided by the facility revealed the following nurse aides (NA) scheduled for the resident census:

Overnight shift:

September 1, 2023, 1 NA for a census of 23, requires 2 NAs.
September 2, 2023, 1 NA for a census of 23, requires 2 NAs.
September 4, 2023, 1 NA for a census of 23, requires 2 NAs.

Interview with the Nursing Home Administrator and Director of Nursing on September 6, 2023, 1:30 PM confirmed the above findings.







 Plan of Correction - To be completed: 09/30/2023

The Facility will do their best to consistently schedule two staff members in an aide capacity for the overnight shift when resident census is over 20 residents. The facility will continue to advertise/market for additional staff members. Ads are currently running on Facebook, Indeed, newspaper, PA CareerLink. The facility is in the process of having two current staff members take the trainings through the department of education to regain our nurse aide certification in order to provide a pipeline for additional staff members. Admissions are on hold at this time. NHA has also contacted local high school to see if students are taking the NA class through the Vo-Tech. Quality Assurance will continue to follow process to ensure compliance
