Initial comments:
Based on an occupancy survey conducted on May 15, 2025, it was determined that Monumental Post Acute care At Woodside Park was not in compliance with the following requirements of the 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.



 Plan of Correction:


§ 205.63(c) LICENSURE Plumbing & Piping Systems-Hot Water Outlets.:State only Deficiency.
(c) Hot water outlets accessible to residents shall be controlled so that the water temperature of the outlets does not exceed 110°F.

Observations:
Based on observations, and staff interviews it was determined the facility failed to maintain water temperatures below 110 degrees Fahrenheit on two resident rooms. (Resident Room 220 and Room 259)
Findings Include:
Observations in the resident room 220 and 259 on May 15, 2025, at 11:00 a.m. revealed the water to be very warm to touch.
Water temperatures were obtained with the Maintenance Employee E4 in the rooms with the following temps on May 15, 2025, at 11:00 a.m.
Room 220 sink- Water temperature- 118-degree Fahrenheit.
Room 259 sink- Water temperature- 118-degree Fahrenheit.
Interview with the Maintenance Employee E4 on May 15, 2025, at 11:00 a.m. confirmed that the facility water temperatures should be below 110 degrees Fahrenheit.



 Plan of Correction - To be completed: 05/30/2025

Water temperature valve has been adjusted to allow more cold-water flow.

Water Temps in rooms 254- 220 were repeated after 1 hour and were at 100 degrees.

Water temperature checks will be done by maintenance in every room daily for 7 days.

Random water temp checks will be conducted by maintenance, 12 rooms and common areas every day thereafter to ensure compliance.

Maintenance will be educated by NHA or designee on the importance of checking water temperature daily to ensure compliance of less than 110 degrees.

Reports will be presented on monthly QA.
