Observations: Name: MAIN BUILDING 01 - Component: 01 - Tag: 0161 Based on observation and interview, it was determined the facility failed to maintain the building construction requirements for the entire building.
Findings include:
1. Observation on March 19, 2024, at 8:45 a.m., revealed the building is a two-story, Type III (200), unprotected ordinary structure, with a basement and attic, that is fully sprinklered. This type of construction is not permitted to be greater than one story in height.
Interview with the Facility Administrator and Maintenance Director on March 19, 2024, at 8:45 a.m., confirmed the building construction type did not meet the requirements for an existing health care building.

1. New owner will have an architect review the blueprint and construction plan of the building in order to determine a solution to correct the deficiency for building construction. 2. The Administrator or designee will monitor progress of correcting the deficiency for building construction. Outcome will be reported to the Quality Assurance and Performance Improvement (QAPI) Committee. Facility will correct deficiency by January 1, 2025.
