Based on a Relicensure Survey completed on October 28, 2025, it was determined that Broad Acres Health and Rehabilitation Center was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy.

This is a one story, Type III (200), unprotected, ordinary building, that is fully sprinklered.





 Plan of Correction:


NFPA 101 STANDARD Subdivision of Building Spaces - Smoke Barrie:State only Deficiency.

Smoke barriers shall be constructed to a 1/2-hour fire resistance rating per 8.5. Smoke barriers shall be permitted to terminate at an atrium wall. Smoke dampers are not required in duct penetrations in fully ducted HVAC systems where an approved sprinkler system is installed for smoke compartments adjacent to the smoke barrier.

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0372

Based on observation and interview, it was determined the facility failed to maintain one smoke barrier separation wall, affecting one of one floor.

Findings include:

1. Observation on October 28, 2025, at 12:14 p.m., revealed a penetration of the portion of the smoke barrier separation wall located within the A Wing, Right Hall Shower Room (located above the suspended ceiling assembly).

Exit interview on October 28, 2025, between 12:45 p.m., and 12:50 p.m., with the Facility Director, confirmed the smoke barrier wall deficiency.



 Plan of Correction - To be completed: 12/08/2025

The penetration of the smoke barrier separation wall located within the A Wing Right Hall Shower room was sealed with appropriate material

Audit of the smoke barrier walls to ensure there are no penetrations with smoke barrier walls

In Service Maintenance staff to ensure integrity of the smoke barrier walls

Random Audits of the smoke barrier walls will be completed monthly for 3 months to ensure there are no new penetrations with smoke barrier walls

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0911

Based on observation and interview, it was determined the facility failed to maintain the electrical systems in one location, affecting one of one floor.

Findings include:

1. Observation on October 28, 2025, at 12:20 p.m., revealed errant wiring lacked a junction box, located above the suspended ceiling assembly, closest to the Vending Machine area.

Exit interview on October 28, 2025, between 12:45 p.m., and 12:50 p.m., with the Facility Director, confirmed the electrical deficiency.



 Plan of Correction - To be completed: 12/08/2025

Added a junction box to errant wiring located above the suspended ceiling assembly, closest to the Vending Machine area to ensure compliance with maintaining electrical systems

Random above ceiling audits will be conducted to ensure proper termination of wiring

In Servicing will be conducted with the maintenance staff to ensure proper termination of wiring

Random above ceiling audits will be conducted monthly for 3 months

Based on a Relicensure Survey completed on October 28, 2025, at Broad Acres Health and Rehabilitation Center, it was determined there were no deficiencies identified under the requirements of the Life Safety Code for an existing health care occupancy.

This is a one story, Type V (111), protected wood frame building, that is fully sprinklered.
