Based on a Relicensure Survey completed on October 22, 2025, it was determined that Berks Meim Nursing and Rehabilitation was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy.

This is a three-story, Type II (111), protected noncombustible structure, with a basement, which is fully sprinklered.



 Plan of Correction:


NFPA 101 STANDARD Emergency Lighting:State only Deficiency.
Emergency Lighting
Emergency lighting of at least 1-1/2-hour duration is provided automatically in accordance with 7.9.

Observations:
Name: NEW NURSING FACILITY - Component: 04 - Tag: 0291

Based on observation and interview, it was determined the facility lacked installed battery back-up lighting, at the transfer switch, affecting the entire component.

Findings include:

1. Observation on October 22, 2025, at 11:05 AM, revealed the ground floor, Electrical Room lacked installed battery back-up emergency lighting, at the transfer switch.

Interview at the time of the exit conference with Administrator and Facilities Manager on October 22, 2025, at 1:00 PM, confirmed the ground floor, Electrical Room lacked installed battery back-up emergency lighting.



 Plan of Correction - To be completed: 11/12/2025

Battery back up emergency lighting installed at transfer switch in electrical room.

Education provided to NHA, Maintenance Director regarding need for emergency lighting of at least 1-1.5 hour duration provided with power outage at transfer switch.

Emergency lighting installed at transfer switch added to monthly 30 second test and annual 90 minute test for emergency lighting.

Will audit monthly x 3 with with findings reported to QAPI.
