(4) The receiving narcotic treatment program shall document in writing that it notified the transferring narcotic treatment program of the admission of the patient and the date of the initial dose given to the patient by the receiving narcotic treatment program.
Observations
Based on a review of patient records, the facility failed to document in writing that it notified the transferring narcotic treatment program of the admission of the patient and the date of the initial dose given to the patient by the receiving narcotic treatment program in one of three patient records.

The findings include:

Twenty seven patient records were reviewed on February 6-8, 2012. Three patients were transferred into treatment from a narcotic treatment program. The facility failed to document in writing that it notified the transferring narcotic treatment program of the admission of the patient and the date of the initial dose given to the patient in record # 5.

Patient # 5 was transferred from a narcotic treatment program and admitted into treatment on July 14, 2011. There was no documentation that the facility notified the transferring narcotic treatment program of the admission of the patient and the date of the initial dose given to the patient.

The clinical supervisor was interviewed on February 8, 2012 and confirmed the findings.

Plan of Correction
A 'Transfer Acknowledgement Form' will be completed for every transferring patient and will be faxed to the transferring clinic. This form will include written notification of the admission of the patient and the date of the initial dose given. A copy of this form will be kept along with the fax-confirmation sheet. This will be done for every transferring patient and will be immediate and on-going. Clinical Supervisor will monitor for compliance.
