INITIAL COMMENTS

This report is a result of an on-site complaint investigation conducted on December 17, 2019 by staff from the Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the on-site complaint investigation, Addiction Medicine and Health Advocate, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

Plan of Correction

705.24 (4)  LICENSURE Bathrooms.

705.24. Bathrooms. The nonresidential facility shall: (4) Provide privacy in toilets by doors.
Observations
Based on a conversation with facility staff, the facility failed to ensure the client's privacy by doors.

The facility failed to ensure privacy because the restroom being used during the weekend is the monitoring restroom where staff would be able to see the client when the client is not scheduled for a urine to be taken.

Plan of Correction
To ensure privacy for patients utilizing the restroom on weekends, when urine drug screens are not typically taken, the monitor in the medication dispensing room will be turned off. This is the only monitor where this restroom can be viewed via closed circuit camera. The Administrative Nurse, in conjunction with the Program Director, will be responsible for communicating with the nursing staff about this matter. The Administrative Nurse will be responsible for ensuring that the nurses keep the monitor off on the weekends, unless there is a need to take an observed urine drug screen.
