This report is a result of a telephone complaint investigation conducted on September 10, 2014 by staff from the Program Licensure Division. Based on the findings of the complaint investigation, the allegations against The Consortium were unsubstantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
