Observations Based on a review of client records, the facility failed to document an annual evaluation of each's patient status that was signed by the medical director.
The client records were reviewed on October 28-29, 2020. Client records #4, #5 and #7 were required to have documentation of an annual evaluation including the required 7 areas as well as the medical director signature. Client records #4, #5 and #7 had documentation of an annual case consultation that did not address all of the required areas, nor did it have the signature of the medical director.
These findings were reviewed with facility staff during the licensing process.
