Based on a review of client records, the facility failed to document the date of the witness signature on consent form in ten of ten records.
The finding include:
Ten client records were reviewed on January 8, 2016. All were reviewed for informed and voluntary consent forms. The facility failed to document the date of the witness signature on the informed and voluntary consent forms in client records, 1 through 10.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Facility Director will oversee that the PCS informed and voluntary consent forms are revised to include a date for the witness signature. The Facility Director will also oversee that staff are informed and educated on the implementation of the revised releases via staff meeting on 2/18/16. To maintain ongoing compliance, the Quality Improvement Department and office administration will ensure that the revised releases are utilized for all clients via chart audits. |
