This report is a result of an on-site complaint investigation conducted on September 12, 2024 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Best of Service CC Y0E46701 was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the Recovery House.

§ 717.29(b)(1) LICENSURE Physical plant standards - Bldg-Exterior-Grnd
§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

Observations
Based on an inspection of the physical plant it was found the recovery house failed to keep the interior of the recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

Plan of Correction
MY plan of correction is to completely bomb theses areas to fully eliminate any rodents clean areas ,fully sanitize to remove supposedly moldings and any and all interior problems.JAMIE SMITH IS RESPONSIBLE ,ENSURING THE CORRECTIVE ACTION IS FULLY IMPLEMENTED. |

Observations
Based on an inspection of the physical plant it was found the recovery house failed to keep the recovery house free of insect infestation.

Plan of Correction
My plan of correction is to Fully Bomb the facility to eliminate completely all pest and rodents that are in facility currently and also for future references do the same.JAMIE SMITH IS FULLY ENSURING ,RESPONSIBLE THAT ALL CORRECTIVE ACTIONS ARE DONE AND WAS COMPLETED SEPTEMBER 12 2024 . |
