This report is a result of a complaint investigation conducted on November 7, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Pyramid Healthcare, Inc. were unsubstantiated and no plan of correction is required.
