This report is a result of a complaint investigation conducted on October 2, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Pyramid Healthcare, Transitions 2 could not be substantiated and the findings were undetermined.
