Plan of Correction
1) The director and clinical supervisor met with the counselors to discuss this citation and how to remedy it.

3) Moving forward, when client files are reviewed by the director or clinical supervisor, this will be something that is thoroughly examined to ensure this requirement is being met.

4) Client records are reviewed weekly at a treatment team meeting. The physicians, nurse practitioner, counselors, clinical supervisor, and the director are all a part of the treatment team which addresses 'where' clients are in their respective treatment plans and if there are any problems. Issues such as this should come up in this meeting.

5) The clinical supervisor will review client files weekly in one-on-ones with each counselor to ensure nothing is missed.
