This report is a result of an unusual incident investigation conducted on July 17, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the unusual incident investigation, the allegations against Discovery House were substantiated. No deficiencies were identified during this investigation and no plan of correction is required.
