The facility's training year that was reviewed was from July 1, 2017 through June 30, 2018 . Employee #2's employee record only documented 7 hours of annual training for the training year reviewed. These findings were reviewed with facility staff during the licensing process.

704.12(a)(6)  LICENSURE OutPatient Caseload

704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios. (a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client. (6) Outpatients. FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients.
Observations
The Staffing Requirements Facility Summary Report was completed and reviewed on May 14, 2018. Employee #6 was hired as a counselor on September 11, 2017 and was still acting in that position. Employee #11 was reported to have 20 hours per week devoted to their 21 clients on their caseload. The counselor Full Time Equivalent (FTE) is determined by dividing the total number of hours the counselor devotes to their clients by 35. Then, in order to obtain the counselor ' s ratio, the total number of clients on the counselor ' s caseload is divided by the FTE.The FTE counselor ' s caseload calculation is as follows: 20/35 = .5714(FTE); 21/.5714 = 36.751, which equals to a client/counselor ratio of 37/1. Employee #7 was hired as a counselor on May 18, 2010 and was still acting in that position. Employee #7 was reported to have 3 hours per week devoted to their 6 clients on their caseload. The FTE counselor ' s caseload calculation is as follows: 6/35= .0857(FTE); 6/.0857= 70.011, which equals to a client/counselor ratio of 70/1.

Plan of Correction
Employee #6 was part time working 3 hours per week and individual to her caseload were added and she did not add hours to her work time in the program.

Employee #11 worked 20 hours in the program and was assigned 21 in the program. This was a mistake on the managers part.

Both of these issues have been corrected and any caseload in D&A will not exceed 35 individuals and casual staff will work 1 hour for every individual on their caseload.

At weekly D&A staff meetings beginning August 7th, caseloads sizes of each counselor will be reviewed along with their hours worked in the program for the past 2 weeks and time scheduled the next week. This will be reviewed, discussed and adjustments needed will be made i.e. re-assigning individuals to other caseloads or increasing scheduled time in program immediately to assure compliance with the requirement.

ADDENDUM July 22, 2019;

We were not utilizing the caseload calculation given to us correctly. This resulted in not having correct caseload ratios for staff not working full time hours.

Employee #6 caseload calculation is as follows; 18/35 = .5714 (FTE); 18/.5714 = 31.5 which equals to a client/counselor ratio of 32/1.

Employee #7 caseload calculation is as follows; 4.5/35 = .129; 4/.129 = 31 which equals to a client/counselor ratio of 31/1.

Caseloads will be reviewed consistently and especially when a client is going to be referred to a counselor who does not work full time in the program. The D&A Program manager will monitor this at assignment, in staff meetings and in individual supervisor using this calculation.

Plan of Correction
The Clinical Services Manager reviewed Treatment Plan Policy #6006 with D&A staff on June 12, 2019. The Clinical Services Manager provided education to all D&A staff that the treatment plan must be reviewed with the individual and updated every 60 days. Meeting minutes were taken of the education.

Staff were given a copy of the procedure for their use. This requirement will also be reviewed during supervision and team meetings monthly beginning in August 2019.

Clinical Services Manager will complete random chart reviews of at least 10 charts per month and audit for timely treatment plan updates.

Plan of Correction
This deficiency was corrected with Procedure 6009 and all D&A staff have been educated on this but there will be formal re-education on August 7th at the weekly D&A staff meeting. Staff have been printing Records of Service regularly since May to place in the clinical file. Files have been caught up with Records of Service in them.

Procedure 6009 has been revised to state that at minimum, the Record of Service is to be printed once per month at the end of the month and placed in filing for the clinical filing. This printing at the end of the month, is the minimum, but these can be printed more frequently and must be printed at discharge.

The Record of Service will be added to the monthly chart audit tool and the Manager will be auditing 10 charts per month to ensure that there is a Record of Service in the clinical file.

The Manager will follow up with staff if the Record of Service is not found in the clinical file to ensure compliance immediately. This audit will occur beginning with July clinical file reviews which will take place by August 30 by the Manager.
