This report is a result of an on-site licensure renewal inspection conducted on August 31, 2022 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Gaudenzia, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

705.6 (7) LICENSURE Bathrooms.
705.6. Bathrooms.
The residential facility shall:
(7) Maintain each bathroom in a functional, clean and sanitary manner at all times.

Observations
Based on a physical plant inspection, the facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times in that the bathroom upstairs flooring was rotting surrounding the drainpipe.
This finding was reviewed with the facility staff during the licensing process.

Plan of Correction
The entire floor is being replaced in this bathroom. We have contracted with a builder to replace the floor in the bathroom. We are awaiting a start date from the contractor. The work is anticipated to be completed no later than 12/31/22.
A daily checklist will be utilized to ensure the proper maintenance of the bathroom. Any issues noted will be addressed immediately. The Program Director will review these checklists on a weekly basis and will document this review by initialing the checklists.
