Based on a review of personnel records, the facility failed to ensure each counselor completed at least 25 clock hours of annual training, during the facility's January 1, 2024 through December 31, 2024 training year, in two of two applicable personnel records reviewed.

Employee # 2 was hired as a counselor on July 12, 2022. The personnel record documented 10.5 hours of training received during the training year reviewed.

Employee # 3 was hired as a counselor on December 19, 2022. The personnel record documented 11.75 hours of training received during the training year reviewed.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Administrator is responsible for ensuring that counseling staff meet the 25 clock hour training requirement. Employee #2 was issued 14.5 hours of training and Employee #3 was issued 13.25 hours of additional training to satisfy the 2024 training year. Both employees will be responsible for completing 25 hours of training in the 2025 training year and each year forward. The Administrator continues to monitor the completion of these trainings and employees are on target to complete the missing training hours by 4/1/2025.
