INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on 5/21/2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, BOJ66701 Za ' Kiyah House 2 was found to be in compliance with the applicable chapters of 28 PA Code Chapter 717 which pertain to the recovery house, therefore no POC is required.
