This finding was reviewed with facility staff during the licensing process.

Plan of Correction
A list of all staff out of compliance with mandatory trainings will be compiled by the 04/01/2015. Staff who are out of compliance will be required to attend upcoming HIV/AIDS and TB/STD trainings in our area in April and May 2015. The head of the Training Department is responsible for implementation and monitoring.

A new release will be created in the EMR system with the 5 perimeters listed in 255.5 by Quality Assurance Manager by 04/05/2015. Confidentiality will be reviewed in a staff training and work groups by clinical supervisors. The appropriate use of releases will be monitored on a daily basis by Medical Records staff.

Client # 2 was admitted to the program on December 15, 2014. The facility failed to document a medical history in client record # 2 as of the date of the licensing inspections.

The findings were reviewed with facility staff during the inspection process.

Plan of Correction
The current medical history used in outpatient is a paper form which needs to be faxed to the Medical Director for review and then faxed back to the office where the patient is being seen. This system is time consuming and fallible. Ultimately the goal is for the form to be incorporated into the EMR system. In the interim, the system will be tightened by eliminating the need to have the form faxed back to the office the patient is being seen. The Medical Records Department will be responsible for ensuring all medical forms are uploading into the patient chart in a timely manner.

709.92(b)  LICENSURE Treatment and rehabilitation services

709.92. Treatment and rehabilitation services. (b) Treatment and rehabilitation plans shall be reviewed and updated at least every 60 days.

The findings were reviewed with facility staff during the licensing process.

Plan of Correction
A revision to our internal auditing procedure is set to roll out by 09/01/2015. With this update, Sigmund, Livengrin's EMR system, will conduct automated audits rather than manual audits conducted by a staff member. This will ensure accuracy and promote a culture of compliance. Timely treatment plan updates will be a quality measure implemented within the new system. In the interim, clinical supervisors and members of the Quality Improvement team randomly audit charts on a weekly basis for the presence of all required documentation. Additionally, every active chart is audited weekly by a Quality Improvement team member to ensure that all documentation is complete. The current and projected audit systems are being overseen by the Quality Assurance Manager.
