INITIAL COMMENTS

This report is a result of an renewal on-site licensure inspection conducted on March 10, 2026 through March 12, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, L9P46701 TNH Sober Living was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.

Plan of Correction

§ 717.30(b)(3)  LICENSURE Safety and emergency procedures - Exits

§ 717.30. Safety and emergency procedures. (b) Exits. The licensee shall: (3) Maintain each ramp, interior stairway and outside steps exceeding two steps with a well-secured handrail and maintain each porch that has over an 18-inch drop with a well-secured railing.
Observations
Based on an inspection of the physical plant, the recovery house failed to maintain outside steps exceeding two steps with a well-secured handrail. These findings were reviewed with the recovery house staff during the renewal inspection process.

Plan of Correction
A Maintenance ticket was created on the day of inspection; the Maintenance Team corrected the handrail on 3/20/2026. This was verified by the Executive Director/House Manager.

At the Resident Leads Meeting on 2/18/2026 the Executive Director informed the Resident Leads of a new ticketing system that allows Resident Leads to submit maintenance tickets for the Maintenance Team to review and complete. These tickets alert TNH Leadership?and are triaged by TNH Leadership?so vital safety concerns can be prioritized and tracked.

The Lancaster District Manager will complete at least monthly rounds of all houses to identify safety and/or physical plant concerns, beginning 3/16/2026. Identified concerns will be reported via the Maintenance Ticketing System.
