This report is a result of an on-site licensure renewal inspection conducted on May 22, 2007 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on June 21, 2007.

709.93. Client records.
(a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following:

Observations
The facility failed to follow its follow-up policy. Client record number 3 did not have a 30 day follow up and client record number 4 had a follow up due in January 2007 that was completed in February of 2007.

Plan of Correction
The process for follow-up contacts was revised to ensure that all of the correct dates were entered and tracked. This will ensure that follow-up contacts will be completed on time. The Outpatient Supervisor will monitor the effectiveness and accuracy of this process. |
