709.33 (a)  LICENSURE Notification of termination.

§ 709.33. Notification of termination. (a) Project staff shall notify the client, in writing, of a decision to involuntarily terminate the client ' s treatment at the project. The notice shall include the reason for termination.
Observations
Based on a review of a patient record, the facility failed to notify the patient, in writing, of the facility's decision to involuntarily terminate the patient.

Client # 1 was admitted on December 1, 2025, and discharged on January 23, 2026. Documentation of the patient being notified in writing of an involuntary termination was missing from the patient record.

Plan of Correction
the Regional Director's sent a memo on March 18, 2026 stating that both a termination of treatment notice and an administrative discharge notice are added to the chart for all involuntary discharges. The administrative discharge form includes details regarding the grievance and appeal process; a copy of these forms must be provided to all patients at discharge. In cases where a patient leaves the property before receiving documentation or refuses to accept it, staff should sign the forms and note the refusal and place them in the clinical chart. Staff will also send a copy of the forms to the patient's personal email address on file to meet regulation. A copy of this email should then be placed in the patient record as proof of delivery. The Program Director is responsible for ensuring these documents are sent, completed and will audit the chart for completion.
