The findings include:

The facility summary report was reviewed on September 4, 5 and 6, 2012. Since the last survey, three out of three staff members were required to complete the HIV/AIDS and TB/STD training. Employee records #21, 22 and 23 did not complete the required training's.

Employee #21 was hired 7-28-2010 as a night counselor. As a support staff member, this employee was required to obtain HIV/AIDS and TB/STD training within the first two years of employment, by 7-28-2012. Employee # 21 failed to obtain the training as of 9/6/2012.

Employee #22 was hired 11-20-2009 as a weekend Activities Coordinator. As a support staff member, this employee was required to obtain HIV/AIDS and TB/STD training within the first two years of employment, by 11-20-2011. Employee # 22 failed to obtain the training as of 9/6/2012.

Plan of Correction
All current staff who have not yet been trained, including the three identified in this POC, will complete this training design by 10/31/12. Subsequent staff hired will complete this training design within the first year of employment.

In order to avoid future violations and ensure that staff persons and volunteers receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum, the Human Resources Department will add those two training designs to the internal spreadsheet which records dates of completion for all required trainings.
