Observations
Based on a review of personnel records and an interview with the project director the facility failed to document ACT 73 reference investigations.Findings: Three of seven personnel records reviewed on May 5, 2010 were required to include documentation of Act 73 reference checks. A request to the project director for that material was made on April 5, 2010. The project director failed to produce the Act 73 reference checks by the exit date of April 7, 2010. Client record # 2. The employee was hired on February 10, 2010. The reference check was required by March 10, 2010. There was no documentation of the Act 73 reference investigation as of April 5, 2010. Client record # 4. A Consultant agreement was signed on September 2, 2009. A reference check was required by October 2, 2009. There was no documentation of the Act 73 reference investigation as of April 5, 2010.Client record # 5. A Consultant agreement was signed on September 15, 2009. A reference check was required by October 15, 2009. There was no documentation of the Act 73 reference investigation as of April 5, 2010.

Plan of Correction
By April 15, 2010, Project Director will have received ACT 73 reference investigation documents from the three applicable staff.
Beginning April 15, 2010, Personnel Director will secure required ACT 73 reference documents on new hires within 30 days from date of hire. |
