INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted from October 23, 2006 through October 26, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on November 17, 2006.

Plan of Correction

709.53(a)(12)  LICENSURE Work as treatment

709.53. Client records. (a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following: (12) Verification that work done by the client at the project is an integral part of his treatment and rehabilitation plan.
Observations
Work therapy was identified in the treatment plans as "Life Skills Training", but the purpose or goal of Life Skills Training was not defined and specific objectives/responsibilities were not included in six of six client records reviewed, #1, 2, 3, 4, 5 and 6.

Plan of Correction
Clinicians have been advised to be more specific and to clearly identify work therapy's intricate relationship to the client's overall treatment goal.

A formal session and follow-up with the clinicians will be conducted by the CEO to review this deficiency Nov. 7, 2006. Upgrading will be instituted immediately. Sign-in sheets will serve as proof for the session.
