This report is a result of a follow-up on-site licensure inspection conducted on April 22, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, SOAR Chester was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The deficiencies were identified during this inspection. SOAR Chester submitted documentation on April 29, 2010 that were verified that needed changes had been made. No further deficiencies are recognized at this time.
