Observations
Based on a review of personnel / training records, the facility failed to document the completion of 25 clock hours of annual training required for counselors in two of four personnel / training records.

The findings include:

Ten personnel / training records were reviewed on December 2 & 5, 2014. Four personnel records pertained to counselors. Only two personnel / training records required the completion of 25 clock hours of annual training as two of the counselors reviewed were newly hired. The facility failed to document 25 clock hours of annual training in personnel / training records # 5 and 10.

Employee # 5 was hired as a counselor on March 5, 2007. The facility training year is from January through December. Employee # 5 completed 7.5 clock hours of annual training from January through December 2013.

Employee # 10 was hired as a counselor on April 8, 2002. The facility training year is from January through December. Employee # 10 completed 22.5 clock hours of annual training from January through December 2013.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
After reviewing the Annual Staff Training Plans, Assistant Director/Clinical and I have decided to begin incorporating a review of staff's training progress into our quarterly Supervison Review Sessions. This will allow for Administrators and Staff to discuss work/training progress and identify areas of deficencies with regard to meeting the required numbers of training hours needed to meet the standards. Including a review of training hours into the regular supervison review sessions will prevent any oversight that may occur and assure that all staff remain on top of where they are with hours.
