INITIAL COMMENTS

This report is a result of an Renewal licensure inspection conducted on May 24th, 2023 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection YMCA Camp Joy was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this inspection and a plan of correction is required.

Plan of Correction

§ 717.27  LICENSURE Notification to family or emergency contact

§ 717.27. Notification to family member or emergency contact. (a) The licensee shall develop and implement written policies and procedures that specify the methods and circumstances, including the resident ' s hospitalization or death, for notifying the resident ' s emergency contact with the resident ' s consent. The resident may revoke consent to provide notice to the emergency contact. (b) The licensee shall make at least one attempt to notify the resident ' s emergency contact in accordance with subsection (a) immediately and in no event more than 12 hours after the resident decides to end residency or does not return to the drug and alcohol recovery house as expected. This subsection shall not apply if the licensee knows or has reason to know of allegations of domestic abuse on the resident by the emergency contact.
Observations
Based on a review of resident records, the recovery house failed to make at least one attempt to notify the resident ' s emergency contact in accordance with subsection (b) immediately and in no event more than 12 hours after the resident decides to end residency or does not return to the drug and alcohol recovery house as expected.

These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
Process of Correction: citation for non-compliance 28 PA Code 717.27

The plan of correction will involve the following: Upon the time of intake/admission the client fills out both the emergency contact release form in addition to an Emergency Medical Information (EMI) form and it is updated regularly, as deemed necessary. We have now also updated and added our new Emergency Notification to Family Member or Emergency Contact form and process at Camp Joy. Upon intake the client and Case Manager will fill out the EMFMEC form and release. If there is an allegation of domestic abuse perpetrated upon the resident by the designated emergency contact, we advise that they use someone else as an emergency contact person.

Once a client leaves Camp Joy early, unplanned, or does not return as expected or in the event of an emergency within 12 hours of not returning to the program when the client would otherwise be expected back then staff will make at least one attempt to notify the client's emergency contact.

After staff has attempted to notify the client's emergency contact then the staff member will fill out the emergency notification reference form that is in the client's file to document the procedure was followed. All of these forms are below and attached.

In addition, file inspection at Camp Joy will be conducted variably during the year to increase accountability.

The individuals responsible for this happening are the Executive Director of Social Services at the YMCA and the Program Director at Camp Joy and we expect all changes and updates to be in effect by 7/1/2023.
