Releases allowing disclosure to a government agency are set up to automatically stay within the limits imposed by 4 Pa. Code 255.5. Additionally, all staff are required to attend necessary Confidentiality trainings. The Program Director will review this regulatory standard, this citation, and the new process/system with all staff at a regular general staff meeting on January 5, 2016. Plan of Correction
We have recently made a full transition to Electronic Medical Records (EMR) and had our system and process designed to eliminate oversights such as the one identified in this citation. The program requires the entry of documentation reflecting cause for any missed or cancelled sessions. This system and process also allows the clinicians and the Clinical Supervisor to identify the need to review and adjust any treatment plans to reflect present actual client need and participation. The Program Director will review this regulatory standard, this citation, and the new process/system with all staff at a regular general staff meeting on January 5, 2016. Clinical Supervisor will monitor records to ensure ongoing compliance. As of 12/15/15 the records involving the services provided to clients #1, #4, and #5 have been reviewed by the Clinical Supervisor and the appropriate clinicans/clients as necessary, and the records have been updated providing explanation or clarification regarding the discrepancies between the treatment plans and the record of services.
