This report is a result of an on-site licensure renewal inspection conducted on June 26, 2007 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on July 25, 2007.

704.11(c)(2) LICENSURE CPR CERTIFICATION
704.11. Staff development program.
(c) General training requirements.
(2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.

Observations
The facility failed to provide sufficient coverage with staff currently certified in CPR for the evening hours. The facility currently has one staff certified in CPR who works the day shift. The regulation requires that the facility provide for at least one person on site during operational hours, however it is recommended that the facility have sufficient staff trained to assure coverage during times of illness and vacations.

Observations
The facility failed to individualize treatment plans in three of four records reviewed. The treatment plans in client records #1, 2, and 4, had the exact same goals.
