Observations
The facility failed to document 25 clock hours of training annually for each counselor.
Staff training records were reviewed on November 2, 2016. The facility ' s training year is January through December and the 2015 training year was reviewed. Employee #4 did not have documentation of at least 25 clock hours of training for the 2015 training year.
Employee #4 was hired as a counselor on October 9, 2000. Employee #4 had documentation of 22 clock hours of training for the 2015 training year.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Director of Women and Children's Programs will review training requirements/standards with Facility Director and Administrative Assistant by 11/21/16.
Administrative Assistant will monitor trainings monthly, and report to the facility director, any staff that are in need of training in accordance with training standards. These monthly audits/training reviews will begin 12/01/16.
All counselor staff will complete at least 25 hours of training each training year. |
