This report identifies the findings of an on-site complaint investigation conducted at February 20, 2007 by staff from the Division of Drug and Alcohol Program Licensure. As a result of the investigation a deficiency was identified and a plan of correction is required by March 23, 2007.

Observations
Documentation of disciplinary actions are to be maintained within the personnel record. Documentation of disciplinary action was missing from the one personnel record reviewed.

Plan of Correction
Jill Wharrey, Human Resource Coordinator, now includes all disciplinary action forms in the personnel file rather than in a separate folder.
