Observations
Based on a physical plant inspection, it was observed that the facility failed to ensure privacy so that counseling sessions cannot be seen or heard outside of the counseling room as the downstairs group room does not have a door, allowing the group to be seen and heard by anyone coming down the stairs.
This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Program Manager, Clinical Coordinator/Supervisor and Contractor met to discuss observation #PO21. Contractor has obtained necessary supplies to rectify the lack of door on the group room. Contractor reviewed plan with Program Manager and this plan will be completed no later than 30 days from submission of this plan of correction. In the interim, all groups will be held in the group room with two doors to insure privacy and confidentiality of all clients. |
