This report identifies the findings of a complaint investigation that was conducted by telephone and administratively November 20 & 23, 2009 by staff of the Division of Drug and Alcohol Licensure. Based on the findings of the investigation, it has been determined that the allegations could not be substantiated. Therefore, no plan of correction is required.
