Observations
Based on a review of personnel records, the facility failed to document job descriptions to include all required duties in two of four counselor records.
The findings include:
Seven personnel records were reviewed on December 11, 2013. Four of the personnel records reviewed were that of counselors. The facility failed to include clinical duties on the job descriptions contained in the personnel records for counselors # 5 and 7.
The aforementioned information was discussed and confirmed by the facility director.

Plan of Correction
Job descriptions for qualified counselors #5 and #7 were ammended on 12/12/13 by the Facility Director to include detailed clinical duties. Counselors #5 and #7 signed the job descriptions on 12/12/13 and the job descriptions were placed in the personnel files of counselors #5 and #7.
Effective Immediately, the revised job descriptions detailing clinical duties for qualified counselors will be used for all new employees. The Facility Director will ensure that the appropriate description is signed by the new employee and placed in the employee's personnel file. |
