Observations
Based on a review of client records, the facility failed to ensure that individual treatment and rehabilitation plans included the frequency of services in five out of five records reviewed.
Client # 1 was admitted on August 23, 2022 and was still active at the time of investigation. Treatment plans dated 8/23/22 and 9/26/22 were missing the frequency of services.
Client # 2 was admitted on September 20, 2022 and was still active at the time of investigation. The treatment plan dated 9/20/22 were missing the frequency of services.
Client # 3 was admitted on August 9, 2022 and was still active at the time of investigation. Treatment plans dated 8/9/22, 9/9/22 and 10/14/22 were missing the frequency of services.
Client # 4 was admitted on July 14, 2022and was still active at the time of investigation. Treatment plans dated 7/14/22, 9/1/22 and 10/12/22 were missing the frequency of services.
Client # 5 was admitted on August 16, 2022 and was still active at the time of investigation. Treatment plans dated 8/16/22, 9/23/22 and 10/19/22 were missing the frequency of services.

Corrective Action Plan:
The frequency, duration, and type of clinical and therapeutic services will be noted in the treatment plans and monitored weekly in scheduled treatment team meetings, as well by the clinical supervisor in supervision with therapist effective November 04, 2022.
