Plan of Correction
On 6/12/2024 the client in question was contacted and a request for them to either go onto the client portal and fill out an ROI for their funding to be billed, or come into the office ASAP, and fill out a ROI on sight. Addiotnally, the client was informed that we needed to do this before anotehr counseling session could occur. The client agreed and to come into the facility to sign a ROI.

In order to prevent this from happening in the future, A training occurred on 6/13/2024 with case managers, completed by the facility Director. A process was developed were a weekly report will be run by case managers to verify any changes in funding for clients. Any new ROI's will be completed with the client at that time, or as soon as the client is able to come in person or sign one on their client portal. This will be reviewed by the facility director as part of the QI process that will occur monthly for the next three months, if the ROI' are obtained as needed, then the QI process will return to a quarterly basis to verify it occurring as needed.

709.28 (d)  LICENSURE Confidentiality

Observations
Based on a review of the client records, the facility failed to document that a copy of a client ' s informed and voluntary consent was offered to the client and a copy maintained in the client record in one out of seven client records reviewed.

This finding was reviewed with the facility staff during the licensing process.

Plan of Correction
The case manager for the Facility in question. contacted Client #2 to have the funding ROI completed as required. Additionally, a new release to the legal entity was completed, being marked that the client accepted a copy in the client portal. The same was done for the family member ROI, so that it was clear the client was offered a copy of the ROI's signed. In order to prevent this from happening in the future, A training occurred on 6/13/2024 with case managers. A process was developed were a weekly report will be run by case managers to verify any changes in funding for clients. Any new ROI's will be completed with the client at that time, or as soon as the client is able to come in person or sign one on their client portal. This will be reviewed by the facility director as part of the QIU process that occurs quarterly to verify it occurring as needed.
