This report is a result of an on-site licensure renewal inspection conducted on August 5, 2014 and August 6, 2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Drug and Alcohol Rehabilitation Services Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

705.6 (2)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (2) Provide a sink, a wall mirror, an operable soap dispenser, and either individual paper towels or a mechanical dryer in each bathroom.
Observations
Based on an observation during the physical plant inspection, the facility failed to provide paper towels or a mechanical dryer and hand soap in each bathroom.

Findings:

A physical plant inspection was conducted on August 6, 2014 at approximately 9:30 AM.

The facility failed to provide paper towels or a mechanical hand dryer and an operable soap dispenser in the first floor bathroom behind the reception desk.

The facility director confirmed the findings.

Plan of Correction
The Facility Director will ensure that the existing paper towel dispenser in the first floor bathroom remains stocked with paper towels at all times.

A permanent soap dispenser will be mounted to the wall adjacent to the sink no later than August 29th, 2014. In the event that the soap dispenser becomes broken or inoperable, pump bottles of hand soap will be placed in the bathroom until the dispenser can be repaired or replaced.

The Youth Care Supervisor will be responsible for daily checks to ensure that the soap dispenser, toilet paper, and paper towels are stocked in the bathroom.

Toilet paper, paper towels, and hand soap were placed in the bathroom on August 5th, 2014, immediately after it was discovered that they were missing.

705.6 (6)  LICENSURE Bathrooms.

Observations
Based on an observation during the physical plant inspection, the facility failed to ensure that toilet paper was provided at each toilet.

Findings:

A physical plant inspection was conducted on August 6, 2014 at approximately 9:30 AM.

The facility failed to provide toilet paper at each toilet in both of the bathrooms on the forth floor residential unit.

The facility director and project director confirmed that there was no toilet paper at each toilet in the bathrooms.

Plan of Correction
Toilet paper dispensers will be installed next to each toilet in both bathrooms of the fourth floor residential unit. This installation will be completed by August 29th 2014. Toilet paper rolls were placed in each toilet stall on August 6th, 2014 following the phyiscal plant tour.

Program staff will perform checks three times daily, during all scheduled bathroom breaks on the fourth floor. Any empty toilet paper dispensors will be filled by the maintenance department immediately. The maintenance department will report to the Facility director regarding toilet paper upkeep.

The facility will be applying for a waiver to the regulation stipulating that toilet paper must be available at each toilet at all times.
