This report is a result of an initial on-site licensure inspection conducted on February 8, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, New Insights II, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
Since this was the initial inspection conducted, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.

Observations
Based on observation during a physical plant inspection, the nonresidential facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well being of clients, employees, and visitors.
The findings include:
A physical plant inspection was conducted on February 8, 2016. The facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well being of clients, employees, and visitors as the door that leads to the second floor balcony exit was damaged. In addition, the basement contained numerous items that needed to be removed.
