This report is a result of an on-site complaint investigation conducted on November 8, 2019 by staff from the Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the on-site investigation, Conewago Place was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

705.3. Living rooms and lounges.
The residential facility shall contain at least one living room or lounge for the free and informal use of clients, their families and invited guests. The facility shall maintain furnishings in a state of good repair.

Observations
The facility failed to maintain the building in good repair. Water stained ceiling tiles were observed on the 2nd floor in the life skills room and the recreation room.

Plan of Correction
The ceiling tiles in the Life Skills office as well as the recreation area on the second floor will be replaced on 11/13/19. Maintenance has been made aware on 11/12/19. Facility Director will follow up with this on 11/14/19. |
