Plan of Correction
To address non-compliance with mandatory communicable disease training, the facility has developed a comprehensive plan of correction. This includes creating a detailed, accessible Standard Operating Procedure (SOP) to guide staff through the registration process. A Training Champion will be appointed to oversee training completion, conduct monthly audits, and report directly to the Executive Director. The existing Smartsheet will be used to track training completion, with hiring managers ensuring new employee details are entered promptly to set a 90-day completion goal, coinciding with the probationary period. It should be noted this refers to being "registered" for the course, not necessarily completing it within 90 days. Monthly audits will assess progress, and a policy will prevent employees from completing probation without finishing the training as outlined in the SOP. To evaluate success, the Training Champion will submit reports detailing completion rates and challenges, discussed in monthly staff meetings. Immediate actions involve developing the SOP and appointing the Training Champion by September 15, 2024, with ongoing data entry and audits beginning immediately. The aim is to achieve a 100% completion rate by November 30, 2024, through continuous follow-up and remediation for staff facing challenges. This structured approach with clear timelines and dedicated oversight ensures compliance and enhances training effectiveness.

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
Based on a physical plant inspection on July 11, 2024, the facility failed to keep the grounds clean, safe, and in good repair. The Department staff witnessed the depletion of water supply resulting in no available water for toilets, showers, kitchen and everyday use. The facility did provide drinking water. Documentation provided by the facility showed this has been an ongoing issue since May 14, 2024.

The findings were reviewed with facility staff during the licensing inspection.

Plan of Correction
To address deficiencies in the maintenance of the building exterior and grounds per regulation 705.2, we implemented immediate corrective actions and established preventive measures. The main water tank's leaking line was repaired by United Well & Pump Services on 7/10/2024, and these repairs were documented by the Maintenance Manager. Future issues will be addressed within an hour by maintenance staff, with semiannual inspections scheduled, starting on 1/10/2025, and weekly monitoring conducted by the maintenance team[TD1] . Monthly reports prepared by the Maintenance Manager will be reviewed by the Executive Director, while weekly reports will track water usage[TD2] . Quarterly education sessions for residents and staff, starting on 8/20/2024, will be led by a Behavioral Health Technician to prevent intentional clogs. We aim for a comprehensive review of compliance and monitoring systems by 9/30/2024, ensuring effective oversight and adherence to safety and sanitary standards. Additionally, regarding the advisement by United Well & Pump services to repair the well in the near future, has precipitated an investigation of the well water performance over in the Engineering Department for their engineering analysis of the well water system/performance testing, we expect the resultant data to be available by 9/30. From this analysis we will be able to assess the viability of the current wells output versus the water demand for the facility.

705.10 (c) (3)  LICENSURE Fire safety.

705.10. Fire safety. (c) Fire extinguisher. The residential facility shall: (3) Ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company. The date of the inspection shall be indicated on the extinguisher or inspection tag. If a fire extinguisher is found to be inoperable, it shall be replaced or repaired within 48 hours of the time it was found to be inoperable.
Observations
Based on a physical plant inspection, it was determined that the facility failed to ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company.

Fire extinguishers located in the dining room, lobby of the dining room, near the rear exit of the annex and near the gym in the basement did not have inspection tags.

This finding was reviewed with the facility staff during the licensing inspection.

Plan of Correction
To address the citation under regulation 705.10 (c) (3), a comprehensive corrective plan for fire safety was implemented. On July 15, 2024, Kistler O'Brien conducted a thorough inspection and tagging of all fire extinguishers, supervised by the Maintenance Manager. Any extinguishers found inoperable will be repaired or replaced within 48 hours. An annual contract with Kistler O'Brien ensures inspections each July, with the next scheduled for July 2025. Monthly internal checks by maintenance staff will verify functionality and tag integrity, with a log maintained for review. Staff will receive annual fire safety training, starting October 5, 2024, facilitated by the Maintenance Manager and Risk/PI Team. Key target dates include completing immediate inspections by August 11, 2024, replacing or repairing inoperable extinguishers by August 13, 2024, and signing an annual inspection contract by August 15, 2024. This plan remediates current deficiencies, enforces routine inspections, and promotes staff awareness, ensuring compliance and enhanced safety. The Maintenance Manager, supported by the Risk/PI Team, is responsible for ongoing oversight of adherence to this plan.

This finding was reviewed with facility staff during the licensing process.

Plan of Correction
To address the citation related to § 709.24 (a) (3), the facility is reinforcing its Non-Routine Discharge (NRD) procedures by requiring all staff to review and sign an updated Standard Operating Procedure (SOP) that mandates notifying the emergency contact within 12 hours of a non-routine discharge, documenting this in Carelogic (EMR), and using a new Smartsheet form (NRD_EC_Hillside) for documentation. The Nurse Manager and BHT Manager will oversee staff training, which is to be completed by September 1, 2024. The BHT Manager will ensure daily form use and, alongside BHT Supervisors, monitor compliance checks starting the same date. Weekly audits by the BHT Manager, beginning September 8, 2024, will assess compliance, with findings reviewed in monthly leadership meetings led by the Executive Director, who will also address any issues promptly. These measures are designed to ensure adherence to client treatment management procedures and enhance coordination in client care and discharge processes.

Plan of Correction
To address the recurring deficiencies in compliance with the Department's approved plans of correction, our facility is implementing comprehensive strategies focused on sustainable improvements. This includes deploying structured tracking systems and dedicated oversight roles to prevent compliance gaps, particularly in mandatory communicable disease training and follow-up information in client records. We've developed a detailed Standard Operating Procedure (SOP) and integrated an advanced Smartsheet system for real-time tracking of training completion among staff. A newly appointed Staff Trainings Champion will oversee the execution of this program, ensuring training is completed on schedule and fostering accountability and continuous improvement. Simultaneously, we are enhancing our client follow-up processes with a new 7-Day Follow-Up SOP, using Smartsheet technology to ensure timely documentation and follow-up within a week post-discharge. Early results of these implementations show promise, streamlining processes and boosting compliance and efficiency. The combination of these initiatives not only remedies immediate deficiencies but also acts as a long-term safeguard against future compliance issues, ensuring our facility adheres to high standards of care and regulatory compliance.
