Observations
The facility failed to display a telephone number conspicuously to the general public for emergency purposes. During a site inspection on May 8, 2019, the facility failed to have a telephone number displayed to the general public for emergency purposes. These findings were reviewed with the facility staff during the licensing process.

Plan of Correction
The Clinical Supervisor has added the agency telephone number to the sign displayed on the front door for the general public for emergency purposes. Any future changes to the sign will include a telephone number listed for reference by the general public for emergency purposes. The telephone number was added to the sign by the Clinical Supervisor on May 10. |
