This report is a result of an on-site physical plant inspection for a physical relocation request conducted on July 13, 2020 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment. Based on the findings of the on-site inspection, Clinical Outcomes Group Incorporated was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.
Since this is a physical plant inspection regarding a facility relocation, not all regulations were reviewed. The facility will be notified by a separate letter of the determination regarding the relocation request. During future inspections, all regulations will be reviewed for compliance.

Observations
Based on a physical plant inspection conducted on July 13, 2020 at approximately 10:30 am, the facility failed to ensure that three bathrooms, located on the basement floor, were ventilated by a working exhaust fan or window.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
By 07/17/20, a letter requesting and exception for not having a ventilation fan in the basement bathrooms will be completed and emailed to DDAP. If the exception is denied then the bathrooms in the basement will no longer be used for staff or clients and all staff and clients will use the bathrooms on the first floor. As of 07/20/20, all staff and clients will use the first floor bathrooms until the exception request has been reviewed. |
