Observations Based on a review of personnel records, the facility failed to provide documentation that counselor assistants were working toward counselor status in two of two employee records.
The findings include:
Eight personnel records were reviewed on April 23, 2013. Counselor Assistant documentation of working toward counselor status was required in two personnel records. The counselor assistants in these instances did not document to the facility director that they were working toward counselor stats.
Counselor Assistant #6 was hired on June 11, 2012. Counselor Assistant #7 was hired June 11, 2012.
The counselor assistants failed to document this information by the end of the 2012 calendar year.
This is a repeat citation from the April 24,2012 licensing renewal inspection.

Plan of Correction For both counselor assistants in question, each met the educational standards for a counselor, but were still needing the 1 year of clinical experience. Although the clinical experiences for both were documented, ARC Manor had no clear documentation on record that stated these 2 individuals were working toward Counselor status. To correct this error, ARC Manor will document for any counselor assistant employed in the month of December how the individual is progressing toward Counselor status. The staff member's supervisor will assess this progress at the same time that they are collecting the individual's annual training needs assessment. Counselor Assistants' progress toward becoming a Counselor will then be documented on the Individual Training Plan for the staff member. This plan of correction will be reviewed with all Clinical Supervisory staff during ARC Manor's May Manager's Meeting. |
