This report is a result of an on-site licensure renewal inspection conducted on September 12, 2007 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Beal Counseling and Consulting was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on October 4, 2007.

709.93. Client records.
(b) The project shall develop and maintain client records on standardized project client record forms.

Observations
Based on a review of client records it was determined that the facility failed to consistently document the personal history form in 2 of 4 client records reviewed. Client record numbers 1 and 4 did not contain the self-administered personal history form. Although the facility does have other documents that include additional personal history information, this form is required by agency policy and contains information that the other forms do not have.

Plan of Correction
A meeting was held with the therapist responsible for client charts 2 & 4 to discuss this issue.
1. The therapist will continue to distribute the blank Personal History at the end of the Intake session.
2. If a client returns for the second session with an incomplete Personal History the therapist will now complete the Personal History with the client present in this session.
3. Clients will also be encouraged to arrive 20-30 minutes before their appointment to complete this document prior to their 2nd session. |
